The mission of the transition homes is to provide healthy, safe, sober, structured and accountable living environments for up to 20 residents as they return to the community. The homes provide a continuum of care for those residents who were involved in the Transformational Living Community (TLC) at Palmer or Hiland Mountain Correctional Centers. Services which are provided within this multi-phased program include case management, substance abuse and relapse prevention services, abuse recovery classes, counseling, family reunification, peer support groups, financial planning, referral services, and post release re-entry services related to employment, transportation, clothing and food.

Residents pay rent of $500 per month which includes utilities, cable TV, internet access, and a significant amount of their food.
